re, continuous EKG monitoring
IV heparin, antiplatelet agent(aspirin) - 평생 복용
*thrombolytic theraphy는 효과 없다.(∵thromgus가 계속 형성되고 분해되기 때문에)
β-blocker, calcium channel blocker
nitroglycerine(→hypotension 주의)
.1∼2일 후에도 증상이 지속되면 coronary arteriography → PTCA/CABG
.내과적 치료로 잘 조절되면 퇴원전에 tredmill test 시행하여
a. ischemia 있으면 PTCA/CABG 시행
b. ischemia 없으면 계속 내과적 치료
Prognosis
.예후는 매우 다양
.adverse events(death, MI, unstable angina 재발)는 대부분 퇴원후 2∼4개월 내에 발생 (단기 예후 불량)
.3개월 내에 20%에서 AMI발생
Variant angina
Etiology
.vasospasm
-주로 근위부 관상동맥(epicardial coronary artery)의 spasm에 의해 발생
-특히 Rt. coronary A.
.3/4 ∼ 2/3 : atherosclerotic coronary artery obstruction을 동반
→ 3/4에서 spasm이 일어난 부위에서 1cm 이내에 고정된 협착이 있음. 운동시 흉통이 발생할 수도 있다.
.1/3 : normal coronary artery → purely nonexertional angina & ST elevation
Symptoms
.다른 협심증에 비해 젊은 나이에 발생(30∼40대)
.resting시, 새벽이나 이른 아침, 또는 잠에서 깨게 만드는 chest pain
.palpitation 이나 shortness of breath가 동반될 수 있다.
.운동과는 상관이 없을 가능성이 높다.
.방실전도장애나 심실부정맥으로 기절하거나 돌연사할 가능성이 있다.
EKG
.ST elevation이 주로 일어남(일부는 ST depression도 일어날 수 있다.)
.T wave의 paradoxical normalization(pseudonormalization)
*exercise EKG test는 가치 없다.
(∵반응이 다양 ; 1/3은 ST depression, 1/3은 ST elevation, 1/3은 변화 없음)
Coronary angiography
.진단에 가장 도움
.provacation test
: Ergonovine, acetylcholine, hyperventilation → vasospasm 유발
.intracoronary vasodilator(NG) → 협착 소실
Treatment
clacium channel blocker(DOC)
nitrate(sublingual NG)
.대체로 치료에 잘 반응하고 예후도 좋다.
.β-blocker는 절대 금기(∵α-receptor가 대신 항진되어 vasospasm 유발할 수 있다.)
.ASA도 high dose는 안됨
